Job Title: Director of Nursing (DON)
Location: Hybrid (Office in Glendale, Field-based in Greater LA & Riverside County)
Reports To: Chief Executive Officer (CEO) or Executive Director  
 

Job Summary:

We are seeking a compassionate and experienced Director of Nursing (DON) to join a leading offsite hospice provider. The Director of Nursing will oversee and coordinate all nursing care services for our patients across the Greater Los Angeles and Riverside County areas. This hybrid role involves a combination of office-based administrative work in our Glendale location and field-based visits to patients’ homes, ensuring the highest standards of patient care are maintained.

As the DON, you will be responsible for leading clinical practices, ensuring compliance with hospice regulations, and providing direct patient care as needed. You will collaborate with interdisciplinary teams, including physicians, social workers, and chaplains, to create comprehensive care plans tailored to each patient's needs. The DON will also be pivotal in maintaining a culture of compassionate care and excellence in service delivery.

Key Responsibilities:

 
  • Patient Care Oversight:

    • Provide direct nursing care to patients as needed, ensuring the highest standards of comfort and quality of life.

    • Oversee the development, implementation, and evaluation of patient care plans in collaboration with interdisciplinary team members.

    • Conduct home visits across the Greater LA and Riverside County areas to assess patient needs and ensure the delivery of optimal care.

  • Compliance & Quality Assurance:

    • Ensure compliance with all federal, state, and local regulations and maintain adherence to hospice and nursing care best practices.

    • Monitor and assess the quality of patient care through regular audits, reviews, and team meetings to ensure consistent delivery of services.

    • Address patient concerns and complaints promptly and effectively.

  • Administrative & Documentation Duties:

    • Complete required paperwork and documentation related to patient care, staffing, and compliance in a timely and accurate manner at the Glendale office.

    • Coordinate with other departments to ensure smooth operational workflows and effective patient care.

    • Participate in regular meetings and planning sessions to maintain a high standard of care and resolve any operational challenges.

  • Collaboration & Communication:

    • Foster strong communication between the clinical team, administration, and family members to ensure coordinated care.

    • Serve as a resource for staff and families, providing guidance on clinical issues and hospice care protocols.

    • Collaborate with the leadership team to improve operational procedures and enhance patient satisfaction.

Qualifications:

  • Education & Experience:

    • Registered Nurse (RN) with a valid, active license in California.

    • Bachelor’s degree in Nursing (BSN) required; Master’s degree in Nursing or Healthcare Administration preferred.

    • Minimum of 5 years of nursing experience, with at least 3 years in a leadership role, preferably in hospice or home health care settings.

    • Strong knowledge of hospice regulations, policies, and best practices.

  • Skills & Abilities:

    • Proven leadership and team management skills.

    • Ability to assess and manage complex patient care needs.

    • Excellent organizational and time management skills, with the ability to manage a hybrid workload (office-based and field visits).

    • Strong communication and interpersonal skills, with the ability to interact effectively with patients, families, and staff.

    • Ability to work independently and be self-motivated, particularly during field visits.

  • Other Requirements:

    • Valid driver’s license and reliable transportation for travel throughout the Greater LA and Riverside County areas.

    • Ability to work flexible hours, including occasional evenings or weekends, depending on patient needs.


Salary range: $90,000-$95,000
