Job Title: Licensed Vocational Nurse (LVN) – Med Pass
Reports To: Wellness Director / Director of Nursing
FLSA Status: Non-Exempt
Position Summary:
The LVN – Med Pass is responsible for safely administering medications and providing basic nursing care to residents in accordance with physician orders and facility policies. The LVN plays a key role in supporting the health and wellness of residents while ensuring compliance with state regulations and standards of care.
Essential Duties and Responsibilities:
- Administer prescribed medications to residents in accordance with state regulations and facility protocols.
- Document medication administration accurately and promptly in electronic or paper medication administration records (MARs).
- Monitor residents for adverse reactions or side e-ects and report findings to a supervising RN or physician.
- Provide basic nursing care, such as wound care, vital signs, and assistance with treatments as directed.
- Communicate effectively with residents, families, physicians, and staff to ensure resident needs are met.
- Ensure proper storage, labeling, and disposal of medications in compliance with facility policy and regulatory standards.
- Participate in care plan development and resident assessments as needed.
- Respond to medical emergencies and initiate appropriate interventions within the scope of licensure.
- Maintain accurate and timely documentation of nursing care and observations.
- Assist with training and supervision of unlicensed sta- (e.g., CNAs/RCAs) as assigned.
Qualifications:
- Current and valid LVN license in California, in good standing.
- Current CPR certification.
- 1+ year of experience in a senior care, assisted living, or long-term care setting preferred.
- Knowledge of state regulations related to medication administration in assisted living.
- Strong clinical judgment, attention to detail, and organizational skills.
- Excellent interpersonal and communication skills.
- Ability to work independently and as part of a care team.
Physical Requirements:
- Ability to stand, walk, bend, and lift up to 50 lbs.
- Must be able to respond quickly to resident needs and emergencies.
