Job Title: LVN- Home Health

Job Type: Regular, 24 hours/week 

Location: West Hollywood, CA 90046

Schedule: Wednesday-Friday 2:00 pm-10:00 pm

Position Summary: 

We are seeking a reliable LVN to join a dedicated home healthcare team supporting a patient with ALS, a tracheotomy and diabetes in West Hollywood. The patient is non-verbal and communicates using an eye gaze communication device. Your role will be critical in delivering consistent, compassionate care and ensuring the patient’s comfort and safety in their home environment.

Key Responsibilities:

• Provide comprehensive tracheostomy care

• Monitor and manage diabetes and administer related medications

• Administer prescribed medications and monitor patient response

• Support feeding tube care and nutritional needs

• Communicate effectively with the patient using assistive communication technology

• Maintain detailed documentation and follow care protocols

• Coordinate with other care providers and family members as needed
• Responsible for hygiene of patient inclusive of bed baths and assistance/cleaning of urine and bowel waste receptacles

What We’re Looking For:

• Licensed Vocational Nurse (LVN) – Active California license

• Previous experience in home health or trach/ALS care preferred but not required

• Strong communication skills in English (fluent) – both verbal and written; essential for interacting with the patient

• Demonstrated reliability, patience, and commitment to long-term care

• Willingness to learn and adapt to specific patient needs

• Ability to work independently and follow medical directives precisely
